Case Management

The work of community case managers includes linking consumers to community resources, identifying and addressing co-occurring disorders, ensuring adequate health care coordination, working closely with the psychiatrist and developing positive relationships with AFC owners.  Through this work, we strive to meet the consumers where they are at and to help them continue moving through the stages of their individual journeys of recovery.
