Residential and Community Support

Our Residential and Community Support programs offer a community setting for those making their comeback.

Unlike in a hospital setting, individuals receive the benefit of professional services while returning to a life of independence.

 

Campus-based residential programs offer two levels of care

24/7 in-home staff supervision

  • Furnished, barrier-free home located on a Hope Network campus
  • Typically for those who have long-term Assisted Living needs

Intermediate supported living

  • Less than 24 hours per day – staff check in throughout the day to assist with daily living skills depending on each individual’s needs
  • On-call staff are available if needed 24 hours per day
  • Barrier-free bedroom and full bath in a four-bedroom apartment on Hope Network’s campus in Grand Rapids

Community-based residential programs offer two levels of care

Community Living Program

  • Client lives in an apartment leased by Hope Network in the community or in his or her own apartment
  • Level of care ranges from hourly checks to just a few hours per week, depending on the individual’s needs
