Post Acute-TransitionalRehabilitation Outcomes

Transitional Neuro Rehabilitation patients are typically admitted to our facility directly from an acute care or general hospital setting, approximately 69 days post-injury.

Patients may have complex medical considerations, but overall they are medically stable. The goal for treatment is to reduce impairments and maximize independence in order to facilitate a safe return to home and community.
