Case Management and Supports Coordination
Managing care is a collaborative process of assessment, planning, facilitation (linking and brokering), care coordination, evaluation, and advocacy for options and services. We are best able to meet an individual’s and family’s comprehensive health needs through communication and available resources. This is done to promote quality, cost-effective outcomes.
Our care management team can assist in care transitions, medication oversight, and even coordinate technology for those who need it. For example, for someone who has undergone a leg amputation, we can coordinate the installation of shower or toilet grab bars and/or house entry ramps.
Ongoing Health Literacy
We can educate in terms people can understand and simplify complex medical information.
Linking and Brokering Community Resources
We can assist people and link them with the community services they need most. Whether they’re unsure of how to do so, or just need to be coached, we can help with medication assistance, transportation, assistive technologies/devices, entitlement enrollment and assistance, home health care services, and supported living.
We link, coordinate and monitor services to promote optimal independence through the Person-Centered Planning process.
We can help with the coordination of services including:
Dietary evaluation and planning
