Transitional Neuro Rehabilitation
Patients are typically admitted to our program directly from an acute care or general hospital setting. Patients may have complex medical considerations, but overall they are medically stable.
Outpatient Services
Therapies designed to improve mobility, strength, cognition and other abilities to perform daily living activities; treatment is customized for each patient based on their rehabilitation goals.
Residential Living
Our residences provide individuals living with a brain injury an opportunity to live in a comfortable home where they can maintain or improve their current level of functioning. Residential settings provide 24-hour supervision and varying levels of support, depending upon the resident’s needs.
Home & Community-Based Services
Routine care coordination is provided to residents living in a Hope Network apartment or in their own home with varied levels of support.
Return to Work Services
Through Return to Work Vocational Services, our program helps individuals reintegrate into the workplace by providing opportunities that help develop and enhance their professional, social, and functional skills.
