Transitional Rehabilitation
Neuro Rehabilitation helps people rebuild the everyday skills that shape quality of life after a serious injury. Through focused, hands-on therapy, individuals practice and regain abilities like moving safely, communicating, thinking clearly, caring for themselves, and returning to meaningful activities at home, work, and in the community.
Our team works together to support each person’s progress. Services may include physical, occupational, and speech-language therapy, along with medical care, emotional and cognitive support, social work, recreation therapy, and nursing—each focused on helping people live as independently and fully as possible.
Day Rehab & Outpatient Services
Outpatient Services support people as they work toward getting back to everyday life. Each person has a care plan designed around their goals, focused on improving strength, thinking, and other skills needed for independence. Therapies can range and may include support for movement, communication, emotional well-being, and returning to work or daily activities.
Residential Living
Our residences provide a supportive, home-like setting where individuals can feel safe, respected, and part of a community. With 24-hour support tailored to each person, residents receive assistance with daily activities, health needs, and mobility—helping them build confidence and live with greater independence and improved quality of life.
Home & Community-Based Services
Individuals living with brain or spinal cord injuries or other neurological conditions may need support to help them live as independently as possible, whether in their own home or in a community setting. Routine care coordination is tailored to each individual, with support ranging from daily periodic checks to 24-hour assistance based on their needs.
Vocational & Return to Work Services
Our Return to Work Vocational Services program supports individuals of all abilities into reintegrating the workforce and achieving their employment goals. Through personalized guidance and real-world opportunities, participants build on their strengths while developing professional, social, and functional skills for meaningful, long-term success.
