If you are not sure where to start, we encourage you to contact our admissions team directly. One of our admissions coordinators will gather some basic information from you and determine the next steps based on your situation. They can assist in obtaining a prescription from your physician, as well as any additional documentation that will be needed.
In order to help make your transition as smooth as possible, our admissions team has prepared a list of basic information they will need to start the admissions process.
- Driver’s license or picture ID
- Name, date of birth, address, phone number
- Health insurance information – ID number, group number, and plan from your HR department
- Auto insurance information – claim number and adjuster name and phone number (if your injury resulted from an auto accident)
- Primary care physician information
- Referring physician information and prescription for service
- Guardian information and documentation (if one has been assigned)
Once you have been referred to Hope Network Neuro Rehabilitation, the admissions team will review your records and funding to determine eligibility. After obtaining funding approval, we will get to work on coordinating a transition plan with you, your family, and your physician.
Hope Network Neuro Rehabilitation offers both inpatient and outpatient programs that are tailored to fit the needs of each individual patient.
Your admissions coordinator will work with you, the referring physician, and your discharge planner to determine which program is the most appropriate for you.
Our Funding Specialists and Admissions Team work collaboratively to secure funding arrangements for brain injury, spinal cord injury and other neurological rehabilitation services through various funding sources including commercial health insurance, workers’ compensation, Auto No-Fault Personal Injury Protection, and other third-party insurances.
Hope Network Neuro Rehabilitation is a contracted provider with many national and state-level payers. Please contact our Admissions Team at 855.407.7575 to review your specific funding situation.
Absolutely. We believe that the love and support of family and friends is the foundation for a successful recovery, so we do our best to integrate their participation in a patient’s care wherever possible. Using personal preferences and treatment team guidance, we lay out a plan that determines how an individual’s support system can be an active part in his or her recovery; not only during therapies but during down times too.
