Residential Services

Each house is staffed with direct support professionals who use the Mandt System to engage with residents one-on-one. A DHHS-approved curriculum is used to assist residents with:

• personal care services
• community living support
• medication administration
• medical training and support

Interpretation services are provided by certified personnel on a contract basis.

Consistent with the culture of gentleness and person-centered planning principles, we believe that everyone deserves to feel valued and to pursue their dreams. To that end, our interventions are informed by individual resident preferences and interests.

We promote self-sufficiency and community inclusion through a number of practices, including posted events calendars with identified staffing supports, individual schedules, and inclusive meetings (with residents). These structures promote predictability, planning, and participation. Many of the individuals living in this home are active in the larger community (within local faith communities, with friendships outside of the home, as active volunteers, and team members).

Enhanced services are available through Hope Network Development and Community Services. The home works with a team of outpatient providers to coordinate care based on individual treatment needs identified in the person-centered planning process. These include:

• Registered dietician for nutritional counseling
• Direct supports professionals using DHHS-approved curriculum for work skills solutions
• Case management-trained supports coordinator for treatment planning and targeted case management
• Social worker (LMSW) for individual therapy
• Behavior specialist (LMSW, LLP, LPC) for behavioral treament planning
• Registered nurse for nursing assessment
• Board-certified psychiatrist for psychiatric evaluation and medication review
