Case Management and Supports Coordination
We provide a collaborative process of assessment, planning, facilitation (linking and brokering), care coordination, educational and school interventions, evaluation, and advocacy for options and services to meet an individual’s and family’s comprehensive health needs through communication and available resources to promote quality, cost-effective outcomes.
Hope Network offers outpatient services for children, adolescents, and adults across Michigan. We offer a comprehensive assessment to identify unique abilities, strengths and weaknesses, mental health and behavioral challenges, personality variables, relationship and family issues, and environmental factors that may be affecting people’s lives. Our experts focus on practical treatment approaches that address each person’s unique needs and abilities. We help individuals enhance the quality of their lives, maximize their potential, learn new skills to optimize their functioning, and improve their relationships.
We have the goal of developing and supporting a family-driven, youth-guided, trauma-informed prevention and behavioral health system of care. Prevention and early intervention services focus on promoting safe and healthy children, nurturing families and strong communities through community and school-based initiatives.
