Health Care Integration and Specialty Interventions

Hope Network Behavioral Health Services provides a continuum of care that’s based on a holistic, trauma-informed approach to help people obtain optimal levels of wellness. We assess each individual and develop a personalized treatment plan that integrates emotional, mental, and physical health.

Care integration promotes both clinically-effective and cost-effective services for a variety of mental health conditions, in a variety of settings. We specialize in Crisis Services, Residential Treatment, and a range of Outpatient Services including Care Management, Therapy, and Prevention Service.

Statewide Continuum

Hope Network provides crisis, residential treatment, community housing, community living supports, case management and supports coordination services, and outpatient therapy services across the state.

Our residential programs provide a continuum of care based on assessment of risk, need, and protective factors. We provide settings for highly acute individuals who frequently discharge to us from psychiatric hospitals, for individuals who have experienced longer periods of stability and are able to operate with moderate levels of independence, along with those who are significantly impacted by both psychiatric and medical conditions. All programs are operated with a trauma focused, relationship centered approach to care.
